a National Health Care Reform  and introduced the Consumer Directed - Home Care Packages 

A home care subsidisation program, putting older Australians in the drivers seat!


Whether you need a little support or comprehensive care, our compassionate care staff will support you from the comfort of your own home.

Home Care Packages are assigned to YOU not the provider.

YOU choose which provider YOU like, then partner with them to help YOU manage the care YOU need.

If YOU are not happy with the provider YOU can end the contract and take any unspent funding with YOU and change to a reliable, affordable, friendly and creditable home care provider, like Carealot.

PLEASE shop around as there are an array of providers with a range of fees and experience.... Some are new comers to the industry while others such as Carealot have assisted 1000's of families for over 24 years.

Register with www.myagedcare.gov.au

Be referred for an assessment to find out whether you are eligible

If eligible, YOU will be told by the department the level of Home Care Package YOU have been approved for.


As an approved provider Carealot would be pleased to be your care partner and assist YOU with coordinating your package of care, services and case management to make certain your needs are met.

Services we can provide

What are the services I am able to buy with the funds?

The support provided through a Home Care Package is a coordinated mix of services that can include:

  • Help with household tasks
  • Equipment (such as walking frames)
  • Minor home modifications
  • Personal care
  • Clinical care such as nursing, allied health and physiotherapy services

Examples:

  • Help with showering, self-care, hygiene and grooming
  • Physical help given to a person and any supervision and prompting of an individual to carry out daily living tasks themselves.
  • Meal preparation and diet
  • Help with feeding, meal preparation, and using eating utensils
  • Foot Care

Examples:

  • Post operative care
  • Wound care
  • Medication management
  • Diabetes education & support
  • Stoma/colostomy/indwelling catheter assistance
  • Care coordination and referrals  for podiatry, physiotherapy, occupational therapy and any other specialised support

Examples:

  • Light housekeeping
  • Meal planning and preparation
  • Grocery shopping
  • Laundry
  • Transporting and/or accompanying you to and from medical appointments,shopping, or other everyday activities.
  • Social interaction and companionship, such as card games, reading, walks, and conversation
  • Overnight care
  • Emergency 24-hour care.

Examples:

  • Maintaining access and egress pathways through a property through
  • Mowing
  • Weeding of established garden beds 
  • Gutter and window cleaning
  • Yard clearance where there are issues of safety and access
  • Essential pruning for access areas
  • Trimming hedges, clearing leaves and sweeping outdoor services
  • Replacing light bulbs and tap washers.
  • Installing batteries in smoke/security alarms
  • Supply of home care aids and equipment
  • Installation of grab rails and ramps
  • Maintenance services including carpentry, electrical, plumbing

  • Pain and symptom management
  • Post-surgical wound care
  • End of life care and support
  • Dementia support
